Writing your Advance Decisions

2. Housing

What kind of housing do you prefer? Do you have a preference for staying in the local community? In your own/family home? If you have to be admitted are there any arrangements that should be taken to keep your home secure and any pets safe?

3. Daily routine and Diet

Is there anything in the general pattern of your day which you want to preserve because they are important or necessary to you e.g. access to cigarettes and a lighter? Any work or activities e.g. access to fresh air, regular quiet time? Any religious or spiritual beliefs or affiliations?

Do you have any likes or dislikes in your diet? Any allergies or special eating times?

do you have any transport or mobility needs?

4. Social Factors

Who do you socialise with? Who plays an important role in your life e.g. family members/friends/church or religious fellowships?

5. Finance

Where do you keep details about your income and savings, and your outgoings? Have you completed an Enduring Power of Attorney and who has copies? If an appointeeship has to be applied for, do you have a preference for who acts as appointee?

6. Other Information

What role do you want your advocate to play e.g. become involved in any assessments about admission to hospital or informed about any changes in your legal status as a patient? Is there anyone you want to be notified in the event of your death being imminent?
