Make sure you end with the following:
- YOUR SIGNATURE AND THE DATE
- DETAILS OF WITNESSES
- STATEMENT AS TO WHETHER YOU HAVE DISCUSSED YOUR DECISIONS WITH YOUR DOCTOR (if so, include details of your doctor
- ANY OTHER RELEVANT STATEMENT
Remember: Your Advance Decision only takes effect when you no longer have the capacity to take decisions about your care and treatment or if you are unable to communicate your wishes. You can make, change or revoke your Living Will at any time, whilst you are of sound mind.
